Use this form to request a Certificate of Insurance when another organization requests a Certificate - usually requested when renting/utilizing another group's property or facilities. Note - individual leaders are NOT authorized to sign contracts for property rental/use which describe the renting organization as Boy Scouts of America or Cascade Pacific Council.
